Due to the current Covid-19 situation Penicuik South Badminton Club is adhering to guidance from the Scottish Government and Badminton Scotland in helping to contain the virus and ensuring a safe environment for players/coaches/volunteers.We are seeking to identify any potential cases at the earliest opportunity to avoid any contact/spread. The research to date for this virus suggests transmission appears to be during the symptomatic phase, and identifying those with symptoms and isolating them should reduce risk significantly.Please answer the following questions YES or NO. This Health Screen Survey should be completed no later than 15th June 2021, or again if returning to the club after a period of illness or if circumstances change.
If the answer is YES to questions 4, 5, 6 or 7 the COVID-19 Officer will contact you.
You may need to seek appropriate medical advice before restarting playing/training. You should not attend the club until this has been done.

5. Have you had a known exposure to anyone with confirmed or suspected COVID-19 in the last two weeks? (e.g. close contact, household member)
6. Have you been asked to self-isolate within the last 14 days?
7. Have you travelled to any country (outside of the UK) in the last 14 days?
If YES, please notify the Lead COVID-19 Officer and check the relevant information for the location and whether 14 day self-isolation is required before returning to training.
